Vaccination Clinic at Barker School

Manitoba Public Health will be offering a vaccination clinic for students, ages 5-11, on March 16th at Lt. Colonel Barker V.C. School from 9:30am - 11:00am. Please note that ONLY students that have an authorized consent form submitted to Barker School Office, from their parent/legal guardian, will be provided the vaccination - 1st or 2nd doses for students, age 5-11, will be available at this clinic. Links to the consent forms can be found in the attached letter. Please complete the attached consent form by clicking on the link below, and submit the consent form to the main office. If you need a hard copy of the form, please contact the school to make arrangements. Unfortunately at this time, due to Covid-19 safety protocols in place, parents are not able to come into school during the vaccination clinic.
